- Get a physical exam, completed on La Verne Athletics Exam Form, from a physician (MD or DO only) .
- The physical exam form to take with you to the doctor can be found in the paperwork packet HERE.
- Please make sure before getting your physical that it is being completed/signed by an MD or DO only. Any physical not signed by an MD or DO will not be accepted. All physical exam forms must be stamped by the physician/physician’s office.
- All physical exams must be dated no earlier than May 1, 2023.
- Complete all physical paperwork/forms.
- The physical paperwork packet can be found HERE.
- Create a SportsWare account and complete the “My Info” section. (New Athletes Only)
- A how-to for signing up for a SportsWare account and completing “My Info” can be found HERE.
- Submit PDFs of completed physical exam and required forms to your SportsWare account. Please submit a clear scan of the form with nothing else in the scan other tha nthe form (i.e> no hand covering the form, objects in background, etc.)
- A how-to for submitting your forms for new athletes is linked HERE and for returning athletes HERE.
- Genius Scan: a free phone scanning app can be found on the Apple Store HERE and the Google Play Store HERE.
- Submit proof of sickle cell trait status on SportsWare. See sickle cell trait form in physical paperwork packet for more information about this step. (New Athletes Only) Please be advised that this step may take time to complete. Please complete ASAP as failure to complete this step will prevent clearance to participate due to NCAA rules.
- Complete yearly baseline ImPACT Concussion Test. (New and Returning Athletes)
- All athletes have received an email from support@impacttest.com with detailed instructions and a link for you to complete the exam.
- Instructions for completing the ImPACT can be found by clicking HERE.
- If taking ADD/ADHD medication, submit required documentation to your SportsWare account.
- The ADD/ADHD medication forms required by the NCAA can be found by clicking HERE.
- If you have had surgery in the last 12 months, submit a clearance note from your surgeon to your SportsWare account.
